What Actually Happens in the First 72 Hours of Alcohol Withdrawal

In my de-addiction practice, I see the same fear on almost every family's face: someone has decided to stop drinking, and everyone assumes the hard part is willpower. The hard part, medically, is the first three days. Here is what is actually happening in the body, because understanding it is what convinces families this needs supervision, not just resolve.

6 to 12 hours: the nervous system starts to rebound

Alcohol suppresses the central nervous system. Stop it after sustained heavy use, and the brain — which has spent months or years compensating by ramping up its own excitatory signaling — is suddenly unopposed. The first signs are ordinary-looking: hand tremor, sweating, a racing pulse, anxiety that feels disproportionate to the moment. Patients often describe this stage as "just feeling off," which is precisely why it gets underestimated.

12 to 24 hours: this is where seizures happen

This window is where withdrawal seizures are most likely, and they can occur in people who have never had a seizure before in their life. 24 to 72 hours: delirium tremens
