Liver enzyme results are one of the most common sources of unnecessary alarm I see on routine checkups. A mildly elevated ALT triggers thoughts of serious liver disease, when the far more common explanation is considerably less dramatic.
What these enzymes actually indicate
ALT and AST are enzymes that live inside liver cells. When liver cells are stressed, inflamed, or damaged, they leak these enzymes into the bloodstream, and levels rise. This is a marker of liver cell disturbance, not a diagnosis of any specific disease — the elevation tells you something is affecting the liver, not what.
Why mild elevations are so common, and usually benign
Fatty liver — now more precisely termed metabolic dysfunction-associated steatotic liver disease — is an extremely common cause of mild, chronic ALT elevation, closely linked to weight, insulin resistance, and metabolic syndrome, and it affects a substantial share of adults in urban populations, most without any awareness. Alcohol, even at moderate levels, can mildly elevate these enzymes, particularly AST. Certain medications, including some very common ones like paracetamol at higher doses, statins, and some antibiotics, can cause mild, usually reversible elevations. Even recent vigorous exercise can transiently raise these numbers, since some of these enzymes are also present in muscle tissue.
The pattern that matters more than the number
The ratio between ALT and AST, and how markedly each is elevated, gives real diagnostic direction. ALT rising more than AST is a pattern more typical of fatty liver or viral hepatitis. AST rising disproportionately more than ALT is a pattern more suggestive of alcohol-related liver injury. Very markedly elevated levels, many times the upper limit, point toward acute injury — from a virus, a toxin, or a drug reaction — and warrant urgent evaluation rather than routine follow-up.
GGT and what it adds
GGT rises with bile duct issues, alcohol use, and some medications, and it is often used alongside ALT and AST to narrow down the likely cause. A pattern of elevated GGT alongside elevated ALT and AST fits alcohol-related change more specifically than either number alone would suggest.
What I actually do with a mildly elevated result
I ask about alcohol intake honestly and specifically, review medications and supplements — herbal and over-the-counter supplements are an underappreciated cause of liver enzyme elevation — check weight, waist circumference, and metabolic markers like glucose and lipids, and generally recommend repeat testing in six to twelve weeks before assuming anything chronic, since a single mildly elevated reading is often transient.
What genuinely changes management
A persistent elevation across repeat testing, a very high single reading, or elevation alongside symptoms — jaundice, dark urine, pale stools, abdominal pain, unexplained weight loss — moves the evaluation toward imaging and more specific testing. Isolated mild elevation on a single unexplained test, in someone without symptoms, is usually not that.
The reassurance worth taking from this
A mildly elevated liver enzyme on a routine panel is common and, in the large majority of cases, reflects something manageable — often weight and metabolic factors — rather than serious underlying liver disease.
